1. HMI Report Policy

CCA (Century Cures Act) requires that medical providers give patients immediate access to significant portions of the patient’s EHI (Electronic Health Information) upon request.

2. Medical Records Release Form

Patients can grant access to their medical records to family members or specific individuals when completing forms. If the patient did not provide such access and is now requesting for someone not initially listed to have access, a form must be completed.

5. Outside Film Form

Houston Medical Imaging regularly receives external CDs from patients scheduled for upcoming scans or when their referring physicians seek a comparison. To maintain a record of the CDs received, it is imperative to complete a designated form and ensure it is stored alongside the external CD.

In most cases, patients will leave with their CD; however, after we store it for 2 weeks if the patient has not yet picked up the CD, we will proceed with mailing it to their home address on file.

6. Medical Excuse

Houston Medical Imaging issues medical excuses upon patient request. It is crucial to accurately complete the form, focusing solely on the date of service, and refrain from providing specific reasons for the visit. This ensures confidentiality and compliance with privacy regulations.

7. Carestream

Houston Medical Imaging utilizes Carestream for image and report storage. This software will be used for viewing and printing patient image CDs.
